How do my dependants access Equipsme benefits?

One of the unique features of Equipsme plans is that members can choose to pay in themselves to add their immediate family, here we explain how family members can access their benefits.

You can add a partner and up to 6 children (25 years and younger) – during the lifetime of your plan, all from your personal Equipsme portal. To find out more about how to add family to your Equipsme plan please read our blog article here.

But what happens when someone on your plan actually needs to USE one of our benefits or services? How do they actually access the benefits they have under your Equipsme plan?

We’ve put together a step-by-step guide below to help you when it comes to making a claim for someone else covered on your plan… but in a nutshell to access the Member's Portal or app, your partner or child can simply use your login details since this is an employee-purchased scheme.

Once they're logged in, they can select themselves as the patient when filling out any forms, etc. They also have the option to contact any of the services directly by providing their Equipsme plan reference number or AXA membership number, both of which can be found on the portal/app.

24/7 GP service

  • All Equipsme plans include our 24/7 GP service, and this is available not only to you but to anyone else you’ve added to your plan.
  • Log on to the Equipsme portal and take note of the Plan Reference Number. Then click on the GP Service icon.
  • When filling in the form to make an appointment, make sure you select the right patient name. Either yourself, or if you are the main member acting on behalf of a child on your plan, you’ll be given the choices of the other names on your plan.
  • Alternatively call the GP service on 0345 222 3712, quoting your Plan Reference Number, and child’s name and date of birth.

Health at Hand

  • The brilliant nurses, midwives and pharmacists can answer all sorts of health questions for all Equipsme members, about all members of your family. Just call 0800 003 004, and quote the main Plan Reference Number.
  • Remember, through the same number you can also talk to specialist heart health and cancer nurses, there to answer questions you might have, whether it’s about the condition, the symptoms, or if you just need reassurance and support about you or a family member who might be ill.

Physiotherapy

  • Physiotherapy sessions are available to all Equipsme members.
  • To make a claim and find an appointment, log on to the Equipsme portal and take note of your AXA Membership Number. Then click on the Physiotherapy icon.
  • When you fill in the form to make an appointment, you’ll also be asked if you’re the patient. Just click ‘No’, and you’ll be given the choices of the other names on your plan.
  • Alternatively call AXA Health on 0800 917 9472 - quoting your AXA membership number and date of birth. Your dependants will be under the same membership number.
  • Remember – always check with AXA to confirm cover before going for your appointment.

Diagnosis and Treatment

  • Diagnosis and/or Treatment may be part of your Equipsme plan depending on the level of cover you have. You can check this on your Equipsme portal to see if they’re part of your package – either as part of what your employer has purchased for you, or as part of your own cover level upgrade. If you are covered, so are your dependants.
  • To make a claim, log on to the Equipsme portal and take note of your AXA Membership Number. Then click on one of the Diagnosis or Hospital Treatment icons.
  • When you fill in the form to make your claim, you’ll also be asked if you’re the patient. Just click ‘No’, and you’ll be given the choices of the other names on your plan.
  • You can also call AXA Health directly on 0800 917 9472 - quoting your membership number and date of birth. Your dependents will be under the same membership number.
  • Remember – always check with AXA to confirm cover before going ahead with any appointments or treatment.

Dental and Optical

Dental and Optical cover is an add-on your employer can choose for you. You can check this on your Equipsme portal to see if it’s part of your plan.

If you do have this add-on you will see the Dental & Optical icon on your portal or app, and if you’ve added family to your Equipsme plan, they’ll ALL get exactly the same amount of cover as you do – at no extra cost. That’s £200 per person per plan year for dental treatment, and £100 for optician costs (plus £25 for eye tests).

This is the only bit of your Equipsme plan where you’ll need to pay up front, get receipts from your dentist or optician, and submit them to be paid back. Click on the Dental and Optical cover icon to submit your receipts electronically.

Stress Support

  • Stress Support is an add-on your employer can choose for you. You can check this on your Equipsme portal to see if it’s part of your plan.
  • Our stress support cover is only for adults. That means your partner and any children over 16 can use the service, too, but not any children younger than 16.

Any one of you can call 0800 917 6479, quoting the scheme number which can be found on your portal or app by clicking on the Stress Support icon.
